Case Packages for a Week of Personal Advocate Services either In Home or via Telephone/Skype
Case packages are offered to promote intensive work on your health and life challenges. The hours are very labor intensive for the client and their family. These packages are designed specifically to bring issues to the forefront immediately and deal with things that are often “danced around” or put aside for another time. The time would be now to ask the questions, walk through all the difficult decisions to see your alternatives and make a plan that can work for all involved. There is no time like the present, so please take advantage of my experience for a total of 25 hours.
Types of cases addressed:
- Chronic care with acute issues
- End of life concerns
- Putting a health book together
- Creating a life plan for your next decade
- Aligning support for your health and life plan
- Making critical changes in your health and wellness
Case parameters:
- Option 1: Five consecutive days in person in the home for five hours each day. This package may also be completed via Skype and telephone with client and family members. Fees may be adjusted if via telephone/Skype, please ask for details.
- Option 2: three days one week and two days the next week in person in the home for five hours each day. This package may also be completed via Skype and telephone with client and family members. Fees may be adjusted if via telephone/Skype, please ask for details.
Cost is based on needs of case. Cost is non-refundable, must be paid up front in full, prior to the first appointment day, and if there are overage hour fees, they can be paid at the end of the five day package. All payments are to be completed at the last hour of the last day. No billing is available nor is insurance accepted. Refunds for the balance of time are available by special circumstance.
Telephone calls are limited and preferably used for changes to the times or urgent issues that arise within the five days. Texts are not to be used other than to request an urgent call back. Voice mails are not to be used other than to request a call back. Emails will be used to communicate completion of tasks for the client, share links for products and services and to briefly state the client’s current condition.
If you have excessive other commitments to family members, work, or other distractions and the project cannot move forward with actions being taken, I have the discretion to discontinue the case or leave the family with templates to continue at their own rate and support them with telephone calls to a maximum to be determined. The fee is nonrefundable if the project is unable to successfully be completed due to other distractions.
The purpose of the case package is to:
- State a problem or concern; how urgent is action needed and who will be participating in the care and support
- Review the current status; assess what type work will be necessary and identify if this is something that is workable for all participating; commitment levels of all
- What has been done in the past and isn’t working; what the current and past stumbling blocks that brought you to the place you are in at present and why have they not worked in the past
- What are you willing to do to change the situation or to support the concerns in life preparation; are we putting a plan in place for others to support you or are we putting a plan in place for you to take action and make better choices; is this something you can do or do you need support
- Define who will be helping the client gather, design and follow through with their health book and life plan; actions will be immediately taken to start this process
- How can steps be taken to change the situation to benefit all involved; particular actions will be required of all the family members
Follow up review will be done two weeks after the case package is completed. This is two hours maximum to see where changes may have to be made, assess the barriers to completion and make new goals if necessary. These two hours are included in the case package pricing. There are times when the goals just cannot be reached and it would be time to make plans to have the client and family reevaluate the issue. While we all want the best results to guide us to good health, at times, it just may not be possible and results may look quite different to each of us. When the case is completed, additional hours may be purchased at the current rate, please ask for details. You may also purchase another case package to advance to the next level of care.
Health and life are not easy to see around the road blocks we seem to hit. Case packages are meant to listen to you, describe who you are, how you are doing and how to improve on it.
